If all articles are not provided at initial submission it will be considered incomplete and will NOT be processed until all articles are received.

 WHO SHOULD APPLY FOR A BIRTH CERTIFICATE?
 Afghan Citizens that need documentation stating their place of birth.

Applications needs to NOTARIZED (Effective June 9th 2014)


2) To be issued a birth certificate you will need to provide your Original Valid Afghan Passport OR Translated and confirmed Taskera from the Ministry of Foreign Affairs Afghanistan. If Valid AFGHAN passport or Translated Taskera is not available, you must apply for a NEW Afghan Passport and complete the requested requirements.

3) Must provide Copies of:

  • Green Card (Front and Back) or If you are a US Citizen a copy of your US Passport
  • Driver's License or Identification Card. (Front and Back)

4) Birth Certificate fees:

  • $10.00 (All fees must be paid with MONEY ORDER ONLY)

  •  NO CASH OR PERSONAL CHECKS ARE ACCEPTED
     Payable to the Order of: Consulate General of Afghanistan in Los Angeles

MAIL: All above requirements are to be sent by CERTIFIED mail, also include a prepaid, self-addressed return envelope. You may use Certified mail, Registered U.S. mail, and Federal Express or UPS shipping envelopes. Tracking number must be present on envelope to assure arrivals to our office; it is your responsibility to track your packages
